Make a referral to LaBase Living
Anyone can refer someone for LaBase Living services — a family member, case manager, guardian, discharge planner, or the individual themselves. The form below takes about two minutes, and we respond promptly. If you'd rather talk to a person first, call (319) 777-8705.
What happens after you submit

Initial inquiry
You send the form or call. We confirm we received the referral and tell you what happens next.

Information and records
We ask, with consent, for the records we need to understand the person's support needs and funding.

Conversation and visit
We meet the person and their team, and visit the home under consideration when relevant.

Decision and transition planning
We share a clear decision and plan next steps together — we'll keep you informed at every step.
It helps to have
- The person's basic information — name, age, and current living situation
- Case manager or MCO contact information, if the person has one
- A sense of which service might fit — and "unsure" is a completely fine answer

Who we serve
Is LaBase Living the right fit?
You don't need to have it all figured out before reaching out. Here's the plain-language version of who our services are built for.
Typical eligibility
- An adult with at least one mental health diagnosis, or an intellectual/developmental disability
- Enrolled in — or working toward — the Iowa HCBS Intellectual Disability Waiver or the Habilitation program
- Living in, or moving to, Eastern Iowa — Cedar Rapids, Marion, Hiawatha, Palo, North Liberty, Coralville, Iowa City, and nearby communities
- A need for ongoing daily support in a home setting — staffed or family-style
Support needs we often serve
- Help with daily living, personal care, and household routines
- Medication support and health appointment coordination
- Support building skills — cooking, budgeting, transportation, employment readiness
- Support with communication, relationships, and community access

Documentation
What we usually ask for later — not now
Nothing below is needed to submit a referral. We request records, with consent, once we're in conversation, so we can confirm we're able to support the person well.
Service and funding documents
- Individual Support Plan or person-centered service plan
- Waiver enrollment or service authorization, if it exists yet
- Guardianship or power-of-attorney documentation, if applicable
- Case manager and MCO care coordinator contact information
Support and clinical information
- Current diagnoses and medication list
- Recent physical exam or health summary
- Behavioral or psychological evaluations, if applicable
- Therapy reports — OT, PT, speech, or behavioral
If you don’t know how to get a particular document, tell us — we can usually point you to the right physician, agency, or case manager and tell you what to ask for.
Funding and referral sources
How referrals and funding usually come together
Services are funded through Iowa Medicaid — the HCBS Intellectual Disability Waiver or the Habilitation program — and coordinated with the person's case manager or MCO.

- Other providers coordinating a move to a less-restrictive setting
