We do not ask for and do not want medical records, diagnoses, or other health information through this website.
We use your information only to respond to your inquiry, evaluate a referral, or consider a job application. Referral information is reviewed by our intake and leadership team. Job application information is reviewed by hiring staff. We do not sell, rent, or trade your information.
We share information only when necessary to provide services or when required by law — for example, with a person's case manager or managed care organization with appropriate consent, with service providers who host our website and store form submissions, or in response to a lawful request. Iowa mandatory reporting obligations always apply.
Form submissions and uploaded résumés are stored in access-restricted systems available only to authorized LaBase Living staff. Résumés are kept in a private storage location that is not publicly accessible. We limit internal access to the people who need it.
We keep referral and contact submissions as long as needed to respond and to meet our recordkeeping obligations as an Iowa Medicaid HCBS provider. Job applications are retained for a reasonable hiring period. You may ask us to delete information that we are not required to retain.
Our website may use basic technical cookies and aggregate analytics to keep the site working and understand which pages people find useful. We do not use this information to identify you personally.
LaBase Living uses one-to-one text messaging between authorized staff and individuals who requested contact, prospective clients, current clients, authorized family contacts, case managers, and referral sources. Messages are used to respond to inquiries, schedule or confirm visits and assessments, send appointment reminders, and provide non-sensitive care coordination updates. Message frequency varies and is typically fewer than 10 messages per month. We do not send unsolicited marketing messages, use purchased lists, or share mobile opt-in information with third parties or affiliates for marketing purposes. Recipients may reply STOP to opt out or HELP for assistance. Message and data rates may apply. Please do not include diagnoses or other medical details in a text message; text is not a secure channel for health information.
Our services are for adults, and this website is not directed to children. We do not knowingly collect information from children.
