Our services: supported community living and host homes

LaBase Living provides Iowa Medicaid HCBS residential services for adults living with a mental health diagnosis and adults with intellectual and developmental disabilities in the Cedar Rapids area: supported community living in our staffed group homes, and host home living with a trained provider family. Both are person-centered, waiver-funded, and delivered by screened, trained direct support professionals. Not sure which fits? Supported Community Living

Supported Community Living

Supported community living (SCL) provides a place to live — a small staffed home — with the daily support a person needs.

  • 3–4 housemates per home, each with a private bedroom

Questions about our services

Supported community living (SCL) is a small LaBase-staffed group home where support levels are set by each person's plan. A host home is family-style living in the home of a trained, contracted provider family. Both are Iowa Medicaid HCBS services.

Yes. LaBase Living is certified for the Iowa HCBS Intellectual Disability Waiver and Habilitation program (Supported Community Living).

Yes — start the referral anyway. We'll point you to the right first steps with Iowa Medicaid and the person's case manager.

If the person does best with structured daily support and housemates, a staffed supported community living home usually fits. If they thrive in a family setting, a host home is often the better match. We'll help you sort it out on a short call.

Yes. People often move between a host home and a staffed home as skills and support needs change — we plan those transitions with the person's team.

LaBase Living uses one-to-one text messaging between authorized staff and individuals who requested contact, prospective clients, current clients, authorized family contacts, case managers, and referral sources. Messages are used to respond to inquiries, schedule or confirm visits and assessments, send appointment reminders, and provide non-sensitive care coordination updates. Message frequency varies and is typically fewer than 10 messages per month. We do not send unsolicited marketing messages, use purchased lists, or share mobile opt-in information with third parties or affiliates for marketing purposes. Recipients may reply STOP to opt out or HELP for assistance. Message and data rates may apply.
