LaBase Living is an Iowa Medicaid HCBS-certified agency providing supported community living in staffed group homes and host home services for adults living with a mental health diagnosis, and for adults with an intellectual or developmental disability, across Cedar Rapids and the Eastern Iowa region. We hold a Three-Year Certification from the Iowa Medicaid QIO and support individuals across our group homes and host homes.

What happens at each step+
- 1. Connect. Call us or send a referral. Tell us about the person in your own words — no forms to gather first, and no obligation.
- 2. Person-centered planning. We learn routines, strengths, and goals with the person and their team, and write a plan in human language around real outcomes — a job, an apartment, a hobby, a friendship.
- 3. Build the right supports. A staffed home or a host home — matched to the plan and delivered in the least-restrictive setting consistent with safety.
- 4. Ongoing support. We keep adjusting as skills grow and circumstances shift. Lives change; plans change with them.
Certification
Certified by the Iowa Medicaid QIO through November 2028
Iowa awards its Three-Year Certification only to providers that meet every standard on review. Ours covers the Intellectual Disability Waiver and the Habilitation program.

The team at LaBase Living met me with patience, not pressure. They helped me rebuild simple routines, get back into the community, and eventually find a job. With their steady support I was able to move into a place of my own — something I didn't believe was possible a year ago. LaBase gave me back my independence, and my dignity.

Is LaBase Living the right fit?

- An adult with at least one mental health diagnosis, or an intellectual or developmental disability
