Getting started

Start with Iowa Medicaid: the person applies for Medicaid and then for an HCBS waiver like the Intellectual Disability Waiver, or the Habilitation program, through Iowa HHS. Once eligible, they choose a certified provider like LaBase Living, and services are authorized through their managed care organization. We help families navigate every step.

An Iowa Medicaid HCBS program that funds long-term support services — like supported community living, respite, and day habilitation — so people with an intellectual disability can live in the community instead of an institution. There can be a waiting list, so apply early.

Iowa's ID Waiver has had waiting lists at times; the Habilitation program typically does not. We respond to referrals promptly with an honest picture of current openings.

Costs & funding

For eligible individuals, services are funded by Iowa Medicaid through the waiver or Habilitation program. In residential programs, room and board is usually paid from the person's own income, such as SSI. We explain the exact split before anyone signs anything.

Our services are built around Iowa Medicaid funding — call us to discuss options.

Care & safety

Every LaBase direct support professional passes Iowa criminal history and child/dependent adult abuse registry checks before working with anyone, and completes mandatory reporter training, CPR/First Aid, and person-specific training before their first solo shift.

Staffing levels are set by each person's support plan and service authorization. Ask us about staffing for a specific home when you inquire.

A designated Care Coordinator is the family's single point of contact. We follow Iowa's incident reporting and family notification requirements, and a designated coordinator keeps families informed.

Staff respond first, then follow Iowa's incident reporting rules. We follow Iowa's incident reporting and family notification requirements, and a designated coordinator keeps families informed.

Referrals

Anyone — family members, case managers, guardians, friends, hospital discharge planners, or the individual themselves.

There are three steps: we receive the referral, we have a conversation and arrange a visit, and then we share a decision and plan the transition. We respond promptly and keep you informed at every step.
