About LaBase Living — care that supports dignity, independence, and everyday life

LaBase Living (LaBase LLC) is a Cedar Rapids, Iowa disability services agency certified by the Iowa Medicaid Quality Improvement Organization to provide Supported Community Living under the HCBS Intellectual Disability Waiver and Habilitation program. Our December 2025 certification review earned a Three-Year Certification — effective through November 30, 2028. We operate staffed group homes and contracted host homes across the Cedar Rapids corridor.

We are locally owned and operated, and we stay deliberately small: staffed homes and host homes most agencies in the corridor do not offer, supported by direct support professionals who know each person by name.

Our approach

What makes LaBase Living different?

Three commitments shape every home, every shift, and every support plan we write.

Person-centered care
Services are shaped around each individual, not the other way around. Every person has an Individual Support Plan built from their own goals, routines, and strengths — and our staff are trained on that plan before their first shift with that person.
Safe support
All direct support professionals pass Iowa criminal history and abuse-registry checks and complete required training, as Iowa law requires.
Community living
The least-restrictive environment, aimed at maximum independence: our members shop, work, worship, and play in the same places their neighbors do.

Where we came from, and where we are now

  1. How we started

    Built by people who did the work first

    LaBase Living was started in Cedar Rapids by people who worked direct support before they ran an agency — and who wanted small homes where staff know each person by name.

  2. December 2025

    Three-Year Certification awarded

    Our Iowa Medicaid QIO certification review earned a Three-Year Certification for the Intellectual Disability Waiver and Habilitation program, effective December 15, 2025.

  3. Today

    Staffed homes and host homes across Eastern Iowa

    We operate staffed group homes and contracted host homes across the Eastern Iowa region and surrounding cities — Cedar Rapids, Marion, Hiawatha, Palo, North Liberty, Coralville, Iowa City, and nearby communities — certified through November 30, 2028.

To improve quality of life through compassionate, personalized support — giving every person we serve guidance, meaningful opportunities, and a real place in their community. “Where care meets community” is not a tagline; it is the test we hold every service to.

An Iowa where every adult with a disability and/or mental health diagnosis has a real home, a meaningful routine, and a valued place in community life — supported by people who know them by name, understand what matters to them, and build support around their goals, choices, and dreams.

Certification

Our certification, in plain language

Iowa certifies HCBS providers through the Iowa Medicaid QIO. Certification reviews examine an agency’s policies, organizational practices, and — weighted heaviest — the actual services people receive. LaBase Living holds a Three-Year Certification for the Intellectual Disability Waiver and Habilitation program (Supported Community Living), effective December 15, 2025 through November 30, 2028.

LaBase Living uses one-to-one text messaging between authorized staff and individuals who requested contact, prospective clients, current clients, authorized family contacts, case managers, and referral sources. Messages are used to respond to inquiries, schedule or confirm visits and assessments, send appointment reminders, and provide non-sensitive care coordination updates. Message frequency varies and is typically fewer than 10 messages per month. We do not send unsolicited marketing messages, use purchased lists, or share mobile opt-in information with third parties or affiliates for marketing purposes. Recipients may reply STOP to opt out or HELP for assistance. Message and data rates may apply.
