Direct Support Professional (Full-Time & Part-Time)

Support adults living with a mental health diagnosis and adults with intellectual and developmental disabilities in our staffed Cedar Rapids and Marion homes — building daily living skills, supporting community life, and documenting care.

Responsibilities

  • Support members with daily living skills — cooking, cleaning, budgeting, hygiene, and transportation.
  • Provide medication support and appointment follow-through as written in each Individual Support Plan.
  • Build trusting, respectful relationships with members, families, and guardians.
  • Document care, incidents, and progress accurately.

Qualifications

  • High school diploma or GED
  • Compassion, patience, and reliability
  • Valid driver's license preferred
  • Ability to pass Iowa criminal history and abuse registry checks

  • Valid driver's license and proof of auto insurance (if driving)

Host Home Provider (Contracted)

Open your home to one adult with a disability and share family life, with training, compensation, 24/7 agency backup, and a coordinator who shows up.

Responsibilities

  • Provide family-style living, daily routines, meals, and community participation in your own home.
  • Support the person's Individual Support Plan goals in everyday life.
  • Keep required documentation and communicate with the Host Home Coordinator.
  • Coordinate scheduled respite and backup care through the agency.

Qualifications

  • All adults in the household pass Iowa criminal and abuse registry checks
  • Home passes a LaBase safety review
  • Completion of LaBase host home provider training
  • A signed provider agreement — no healthcare background required

Documents required

  • Photo ID for every adult in the household
  • Consent to background and abuse registry checks for every adult in the household
  • Proof of homeowner's or renter's insurance
  • Completed home safety review checklist

Case Manager

Coordinate person-centered services across our homes and in-home programs, working with members, families, guardians, and managed care organizations.

Responsibilities

  • Maintain and update Individual Support Plans with members and their teams.
  • Serve as the point of contact for families, guardians, and MCO care coordinators.
  • Review documentation for accuracy and follow up on service authorizations.
  • Support intake, referrals, and pre-placement visits.

Qualifications

  • Bachelor's degree in a human services field, or equivalent experience
  • Experience with Iowa Medicaid HCBS services preferred
  • Strong written documentation skills
  • Valid driver's license and ability to pass required background checks

Iowa requires criminal history and child/dependent adult abuse registry checks for anyone working with the people we serve. We run these before your first shift, at no cost to you.

Full-time, part-time, weekend, and overnight shifts are available across our Cedar Rapids and Marion homes. Host home provider roles are contracted rather than shift-based.
